RULES AND RESTRICTIONS

LIFELINE IS A GOVERNMENT BENEFIT PROGRAM
ONLY ELIGIBLE CONSUMERS MAY ENROLL IN THIS PROGRAM
THIS PROGRAM IS LIMITED TO ONE BENEFIT PER HOUSEHOLD, CONSISTING OF EITHER WIRELINE OR WIRELESS SERVICE
BY LAW, THE LIFELINE PROGRAM IS ONLY AVAILABLE FOR ONE PHONE PER HOUSEHOLD
CONSUMERS WHO WILLFULLY MAKE FALSE STATEMENTS IN ORDER TO OBTAIN THE BENEFIT CAN BE PUNISHED BY FINE OR IMPRISONMENT OR CAN BARRED FROM THE PROGRAM

PROGRAM-BASED ELIGIBILITY
PROVIDE DOCUMENTATION SHOWING THE CONSUMER, ONE OR MORE OF THE CONSUMER’S DEPENDENTS, OR THE CONSUMER’S HOUSEHOLD RECEIVES BENEFITS FROM ONE OF THE FOLLOWING FEDERAL ASSISTANCE PROGRAMS:
• Medicaid
• Supplemental Nutrition Assistance Program (Food Stamps)
• Supplemental Security Income
• Federal Public Housing Assistance (Section 8)
• Low-Income Home Energy Assistance Program
• National School Lunch Program (free lunch program)
• Temporary Assistance for Needy Families; or
• Any other program that meet eligibility criteria established by a state for its residents

INCOME-BASED ELIGIBILITY
PROVIDE ONE OF THE FOLLOWING FORMS OF DOCUMENTATION SHOWING ANNUAL HOUSEHOLD INCOME AT OR BELOW 135% (OR HIGHER, DEPENDING ON STATE) OF THE FEDERAL POVERTY GUIDELINES:
• Prior year’s state, federal, or Tribal tax return
• Current income statement from an employer or paycheck stub
• A Social Security statement of benefits
• A Veterans Administration statement of benefits
• A retirement/pension statement of benefits
• An Unemployment/Workers’ Compensation statement of benefit
• Federal or Tribal notice letter of participation in General Assistance; or
• A divorce decree, child support award, or other official document containing income information.
